Introduction
Hi, I’m Liza Merriment. I’m a Naturopath, Nutritionist and Herbalist. I am also certified as a Ketogenic Diet & Health Coach and a Health & Nutrition Life Coach. I am trading as Jervis Bay Naturopathy and my ABN is 40 326 465 579.
My services are suitable for anyone that is looking for a natural solution to improve their health and is willing to follow a personalised plan. My services are NOT suitable for people who are looking for a quick fix and who are not willing to put in some work to reach their goals.
I am passionate about the benefits of a Ketogenic diet, Metabolic Autophagy, Fasting and teaching people how to take charge of their own health. I love helping my clients to reach their goals using a combination of science and herbal medicine.

This document sets out the Terms and Conditions you need to be aware of when purchasing my services. Please take a moment to read it carefully as it sets out your important rights and obligations and I care about making sure we both know where we stand. When you use my services you agree that you are over the age of 18 and willing to be bound by these Terms and Conditions. If you don’t accept this agreement, you should notify me immediately so we can discuss your concerns. My services are offered in compliance with Australian Consumer Law.

My Services & Responsibilities
This disclaimer relates to Ketogenic Diet & Health Coaching and Naturopathic Services.
I tailor plans specific to my client’s health history, body type, goals, likes and dislikes. I guide them through any problems and keep them motivated and accountable so they may reach their health goals. I have an Advanced Diploma of Integrative Natural Health (Naturopathy), a Certificate in Ketogenic Diet & Health Coaching and a Certificate in Health & Nutrition Life Coaching. I am also an Accredited Fitgenes Practitioner and GEMM practitioner.
Your Responsibilities
It is your responsibility to complete your health history intake questionnaire as detailed as possible. I cannot be held responsible for the results incurred from information you have neglected to tell me. You must inform me immediately of any side effects or problems that arise during your treatment, so that I may help you as quickly as possible.
If I prescribe something that doesn’t feel comfortable to you, please contact me to discuss it.
It is also your responsibility to seek independent advice from your doctor before using my service. I am providing information that is not a substitute for medical advice for your specific circumstances.
Potential Issues
Major weight loss can lead to a temporary rise in LDL cholesterol. For this reason, it could make sense to wait until your weight has been stable for a few months before assessing your cholesterol levels.
If you have been on a low fat diet for many years, you could possibly have gallstones that are dormant in your gallbladder. Increasing your dietary fat intake may stimulate your gallbladder to start working again and could possibly aggravate dormant gallstones.
I aim to produce your plan within 1 week of receiving your intake questionnaire, however, due to unforeseen circumstances it could take longer.
Results
I have studied extensively to gain knowledge about the ketogenic diet and its effects on various illnesses but I do not claim to cure any cause, condition or disease. Results from a Weight Loss plan will vary from person to person. No guarantee can be made as to weight loss numbers or health changes. I will work with you to help you achieve your goals. However, your results will vary depending on your medical conditions and your adherence to the plan.
Limitation of Liability
You agree to indemnify me (hold me blameless) for any injury or illness that you might suffer or any damage or loss that might occur while we are working together. If you have any doubts at all about any issue that might arise, you agree that you will conduct your own research and make your own fully informed decision about what is best for you.
In no event shall I or Jervis Bay Naturopathy be liable to you for costs, loss or damage of any kind arising out of or related to this Agreement or the services provided in relation to it. If this clause is unenforceable for any reason, my total cumulative liability for all causes of action of any kind shall not exceed the total amount you have paid me for my services.
Intellectual Property
The custom plan that I write for you and the information that I give you are protected by copyright. They are for your personal use only and may not be copied or shared in any way without my written consent.
Confidentiality
I will collect personal information about your health history and current health including diet, exercise, illnesses, family history, personal health issues and personal goals. This gives me a clear understanding of your current health status will help me to provide the most suitable plan for you.
I cannot provide you with my service if you do not provide me with adequate information. I collect the information via the intake questionnaire, in face-to-face consultations, via email, phone and SMS. Your personal information will be stored securely and kept completely confidential. Your email will automatically be added to my Monthly Newsletter subscription list in Mailchimp 9which you are able to easily unsubscribe from). It will not be shared with anyone else.
Payment Terms
“Diet & Health Coaching Packages may be paid in one of 2 ways
1. An up-front payment by Mastercard, Visa, Eftpos, EFT, Afterpay (in-clinic only) or cash
2. Payment plan of weekly direct debits via Stripe. The Stripe fees will be paid by Jervis Bay Natutropathy.
All Infrared Sauna sessions, supplements and herbal mixtures are at an additional cost unless specified in your package. Credit card details are not retained by Jervis Bay Naturopathy. All prices are in AUD.
Delivery of Services
1. If you would like to ask any questions before you book your initial consultation, please contact me via phone, email or in person
2. After you book your initial appointment, you will receive by email a questionnaire to fill out. I must receive this before your first appointment
3. At your initial appointment, we will further discuss your health. You will be removing your shoes and socks for the Bio-Impedance scale
4. At your second appointment, you will receive your plan
5. At your weekly check-in appointments, I will perform a BIA scale reading (if in person) and we will discuss your progress.
Missed Sessions
My goal at Jervis Bay Naturopathy is to provide quality health care in a timely manner. Therefore, you must notify me if you are unable to attend your appointment no less than 24 hours prior to your scheduled appointment. To assist with this, you will be sent a reminder of your upcoming appointment 48hrs prior to your allocated time.
Please note that if you cancel, miss an appointment or reschedule within this 24hr period, 100% of the consultation fee will be charged. This means that if the appointment was part of a “package” you have forfeited that appointment.
Refund Policy
Diet & Health Coaching Packages – If you change your mind, I offer a full 100% refund up until your first appointment. After that time, work will have begun on your plan and you are no longer entitled to a refund if you change your mind.
Please contact me to discuss whatever problem you are having with the package and I will attempt to negotiate a win/win solution. If you cancel your package, I offer credit to be used on my services within 12 months of the package purchase date. Credit is not transferable to another person.
Fitgenes Testing – Once your sample has been posted to Fitgenes, you are no longer entitled to a refund if you change your mind. The initial appointment and testing fees are payable.
Consumer Guarantees
If I need to reschedule your appointment due to unforeseen circumstances, I will reschedule to a time that suits us both.
If a supplement I have ordered for you is out of stock with the supplier, I will keep you informed of it’s expected delivery.
Results from a Weight Loss plan will vary from person to person. No guarantee can be made as to weight loss numbers or health changes.
I do not claim to cure any cause, condition or disease.
I comply with Australian Consumer Law. If you believe there is a major problem, please contact me to discuss the situation so we can work out a win/win solution.
Jurisdiction & Dispute Resolution
Jervis Bay Naturopathy in located in NSW. This agreement is subject to the governing law of NSW
If you have any issue or complaint arising out of your use of the packages provided or of this client agreement, I agree to make a genuine effort to resolve the dispute through negotiation and discussion.
If we are unable to resolve a dispute by negotiation and discussion within 14 days, the parties must proceed to mediation with the assistance of an accredited mediator who is independent of the parties. The mediator is to be appointed by agreement of the parties or, failing agreement within twenty-one (21) days of the first notification of the dispute, by a person appointed by the Chair of Resolution Institute, (ACN 008 651 232, Level 2, 13-15 Bridge Street, Sydney NSW 2000; telephone: 02 9251 3366, email: infoaus@resolution.institute) or the Chair’s designated representative. The Resolution Institute Mediation Rules shall apply to the mediation. We agree to share the costs of mediation equally between us.
It is a condition precedent to the right of either party to commence litigation other than for interlocutory relief that it has first offered to submit the dispute to mediation. Litigation is to be considered a last resort and may not be commenced until, in the opinion of the independent mediator, the potential for negotiation and mediation have been exhausted.
